Dance

• READ CAREFULLY AND ACCEPT BELOW I hereby indemnify, release and forever hold harmless the Sand Witches of New England, King Richard’s Faire, and Soulshine Media, including its owners, and dancers from any and all claims of liability arising from any accident, personal injury, death, property loss or damage sustained by myself/and minor child(ren) for whom I am a legal guardian (the “Participant”), while the Participant is engaged in activities connected with the performance of the Witch Dance at King Richard’s Faire. I understand that dance activities have inherent risks of injury, and, being fully aware of all risk, I accept full responsibility for providing adequate health and accident insurance coverage for the protection of the Participant. By accepting and consenting to this statement, I declare that the Participant is in good health, with no physical conditions that might prevent participation in the performance. I am aware of the Participant’s physical limitations and agree not to exceed them. Further, I authorize and agree that Sand Witches of New England, King Richard’s Faire, and Soulshine Media may take and use photographs/videos of me and/or the Participant for purposes of record keeping, advertising, and marketing. I understand that I do not have any rights to these photographs/videos and will not be compensated for the same. I have carefully read the contents of this waiver form and fully understand its contents. I understand that this is an assumption of risk and release of liability, and I accept it of my own free will.
